If you are unsure of your insurance coverage, we are happy to check insurance benefits for you prior to your first appointment. We will directly bill your insurance company so there is no additional paperwork for you to complete.

Our clinicians are paneled with many insurance companies. If you participate with a company that is not listed below you may still qualify for full reimbursement of counseling services.

We have flexible options for those that do not have insurance coverage and those with high deductible plans. Please email info@MomentumCounselingServices.com for more information.

Payment 

Credit, Debit, HSA, FSA, Cash, and Check are all accepted for payment.

Cancellation Policy

If you do not attend your scheduled appointment and you have not notified us at least 24 hours in advance, you will be charged a $75 cancellation fee. This fee is not covered by insurance. Exceptions are made for emergencies, sudden illness, and prohibitive weather conditions.

**Please note that insurance benefits are an agreement between you and your insurance provider. We cannot guarantee coverage or reimbursement. If you have eligibility questions, you should contact your insurance provider directly by calling the number on the back of your insurance card.

Currently, we are offering in-person, telephone, or video conferencing through our secure online portal. If you have already seen a therapist at our clinic, please contact them directly. If you are a new client and would like to set up an initial appointment, please call 801.890.0737, leave a message, and we will contact you as soon as possible to find the best fit and format of service for you and your family.
